To ensure availability for your wedding day, please return this form as soon as possible. Once this form and a non-refundable booking fee of $100 that will go towards your final balance is received, your appointment will be officially booked. The $100 booking fee will be deducted from the remaining balance the day of the wedding.
Cash is preferred, but all major credit cards are accepted as payment.
On the day of the service, please arrive with a clean face with a little bit of moisturizer.
I agree to the MINIMUM BOOKING FEE of $650 (Bride + 3 Bridesmaids or Guests) (Does NOT include travel fee). If you do not meet the minimum booking fee, the fee still applies. You can use the credit towards additional guests. An invoice will be shared and the full payment is due the day before the wedding. We accept cash payments on the day of the wedding. Also, cash tip for our artist is greatly appreciated.
Travel fee is additional and dependent on the customer's location. Customer is responsible for all parking fees that may be required at the location. Travel fee is due the day of the wedding with the remaining balance.
In the event you need to cancel your contract for any reason within 60 days of your wedding date your credit card listed below will be charged the minimum booking fee of $650. In the event I am unable to perform services due to any reason and no other assistants are available. $100 booking deposit will be refunded to your account.

I take hygiene seriously. All makeup products and brushes are kept sanitary. All products are sanitized between every makeup application. Any skin condition should be reported prior to the application. Client agrees to release the artist from liability for any skin complications due to allergic reactions.
